Who Is Eligible?

Our members include the Florida Hospital employees, Florida Hospital medical staff, Florida Hospital auxilians, employees and retirees of Adventist Health Systems, employees and retirees of Florida conference and southeastern conference of Seventh-Day Adventist, faculty, staff, and students of Adventist University of Health Sciences, Seventh-Day Adventists church members that live, work, or worship in Orange and Seminole Counties and immediate family of existing members.

How to Apply?
Applying for membership is easy. You can apply online by clicking here. Or simply print out and return the membership application with the items below:

  • Copy of Florida driver’s license or ID
  • Copy of social security card
  • Proof of Membership Eligibility (i.e. Hospital I.D. badge)
  • Minimum opening balance of $25.00 for your Savings account

*If you are eligible through the Seventh Day Adventist Church, then please provide an official letter from the church stating that you are a member of that church.

You may bring in your completed membership application into any branch location along with the required documents or mail to:
